Application for Death Certificate
For people who will be coming to the Pettis County Health Center in person, please use the form below by clicking on the link. Also don’t for get ID and copies of these records are $13.00 each. Additional copies of the same death record ordered at the same time are $10.00 each.

For individuals who will be mailing in for a Death Certificate to the State of Missouri, please use the form below by clicking on the link and follow instructions very carefully.

Ways to Obtain a Certifed Copy of a Birth or Death Record

 

Who May Receive Copies of Death Records

    • All immediate family members. (The term immediate family shall include those family members and in-laws in the direct line of descent up to, but not including cousins. ~19 CSR 10-10.090)

    • Genealogists representing family members

    • Professionally recognized genealogists

    • An attorney acting on behalf of the registrant or his/her family

    • A physician acting on behalf of the registrant or his/her family

    • Authorized agent acting on behalf of the registrant or his/her family; an authorized agent shall produce a signed notarized statement by the registrant or a member of his/her family authorizing the release of record to the applicant

    • A funeral director may act as the official representative of a family to obtain copies of death certificates only.

    • Others may demonstrate a direct and tangible interest when information is needed for determination or protection of personal or property rights. (with third party form)

    Who May NOT Receive Copies of Death Records

    • Friends

    • Cousin

    • Attorneys who are not acting on behalf of the registrant or his/her family (Request must be accompanied by court order)

    • Physicians who are not acting on behalf of the registrant or his/her family

    • Authorized Agents who do not produce a signed notarized statement from registrant or a member or his/her family

    • Others who cannot demonstrate a direct and tangible interest for protection of their own personal or property rights

    • Representatives of heir-tracing companies/news media that do not comply with restrictions set forth

    • Genealogists are only eligible for copies of birth records if they also are eligible as an immediate family member or meet one of the other criteria as described previously
